Quality indicators for hospitals and clinics in Switzerland:

Hip replacement (artificial hip joint), reoperation

frequency of necessary, premature reoperations

Newly implanted artificial hip joints should allow patients at least 15 to 20 years of pain-free mobility. Unfortunately, in an average of 2.5% of patients, the first operation is not successful in the long term. In these patients, a second operation (hip joint revision) is necessary within two years. The reasons for this are usually infections, loosening of the stem or fractures close to the prosthesis. Re-operations should be avoided if possible, as they are much more likely to be unsuccessful.

The proportion of such premature reoperations - and thus the risk for the patient of having one - is not the same at all hospitals. If you need an artificial hip joint, you may therefore find it advantageous to consider the 2-year re-operation rates when choosing your hospital.

In order to determine the hospital- or clinic-specific reoperation rates, hip surgery patients in Switzerland were observed over several years. The figures from this nationwide implant register can be considered very reliable. Almost all hip prosthesis operations performed in Switzerland (almost 20'000 artificial hip joints per year) have been recorded. Informative data are available for 119 hospitals and clinics or 63'960 operations.

Particularly relevant for patients is that eleven hospitals/clinics have significantly better outcomes, i.e. fewer 2-year reoperations (green «scores» in the best list). Two hospitals/clinics are even highly significantly better . These are the Sonnenhofspital (Lindenhof AG) in Bern, as well as the Schulthess Klinik in Zurich. The latter has an impressively low re-operation rate of 1.1% with almost 1'000 hip prostheses per year. As with the good results of the smaller clinics, a bit of luck may be involved, the Schulthess Clinic is the real front-runner for us.

Significantly worse i.e. higher re-operation rates were found in fifteen hospitals (red «scores» in the best list). Three hospitals are even highly significantly worse: these are the Salem-Spital (Hirslanden Bern), the Klinik Permanence (Hirslanden Bern), the EOC Bellinzona and the university hospital Centre Hospitalier Universitaire Vaudois (CHUV) in Lausanne. The 2-year reoperation risk in these hospitals is many times higher than in the best hospitals.

About the indicator «Re-operation rates»

Mesuré en tant que taux de réopération ajusté au risque pour les premières implantations d'endoprothèses (hanche artificielle, genou artificiel) dans les deux ans - par hôpital.

Strengths:

Re-operations after the insertion of prostheses are risky for patients and are associated with additional suffering. A low re-operation rate provides a reliable indications of optimal surgical techniques and procedures, the use of the best possible prosthesis products, and high-quality follow-up care.
The re-operation rates (implant register) are risk-adjusted by the Foundation for Quality Assurance in Implant Medicine SIRIS with various additional measurements (age and gender, BMI, ASA-Sore and Charnley classification). This elaborate form of risk adjustment meets high scientific standards.

Limits:

Re-operation rates are only meaningful when a hospital has a sufficient number of cases. In the case of small hospitals, nothing can be said with certainty (whereby small case numbers are considered unfavourable in terms of good quality anyway). In order to achieve statistically sufficient case numbers, observations are made over two years. This means that the figures are already two to four years old when they are published. It is therefore possible that a hospital that performs poorly could have its quality improved in the meantime and vice versa.

The surgeon "hit it too hard" resulting in a fissure in the femur (TEP). Furthermore, had an ISG blockage during surgery, which, however, was not diagnosed in the Schulthess by either the assistant doctor or the professor. I couldn't sit or walk for months.

Hospital Burgdorf and Langnau are not recommended! Unprofessional staff, even the chief physicians. Worst of all in the hip orthopedia. Better go to the Lindenhof group. Surgery and care were unprofessional, with me and various of my relatives

Artificial hip joint. Pan was completely incorrectly set, and several muscles were damaged. Overdose on painkillers .. and I probably fell off the operating table, the whole right chest and shoulder area was blue after the surgery...Had to bear massive pain!!!
